The giant neuroendocrine tumor with thoracic and abdominal localization

The case of nontypical localization of giant low differentiated neuroendocrine tumor in posterior mediastinum with involvement of retroperitoneum. Given low differentiation of the tumor, for the first step of treatment patient received 3 cycles of polychemotherapy in EP regimen (etoposide+cisplatin) without significant clinical response. For the second step thoracophrenolaparotomy, removal of tumor in posterior mediastinum and retroperitoneal space with diaphragm resection and atypical resection of lower lobe of the left lung were performed. During revision there were no signs of great vessel involvement. Given radical surgery and morphologi structure of the tumor, adjuvant chemotherapy was not required. The post-op follow-up was – 14 months with no signs of disease progression. This case shows the opportunities of surgery in the treatment for giant neuroendocrine tumors of difficult anatomic areas of adjacent regions.
