Long-term results of surgical treatment under extracorporeal circulation in patient with pulmonary artery leiomyosarcoma

Diagnostics, clinical course and surgical treatment outcome in patients with pulmonary artery leiomyosarcoma are reviewed.

Materials and methods. The female patient, 44 years old, was admitted to the department of lung and mediastimum surgery of Russian scientific surgical center named under acad. VB Petrovskiy RAMS, 09.03.2005 complaining of cough with mucous expectoration in small amounts, dyspnoea on slight exertion, palpitations. After the full examination the diagnosis of leyomiosarcoma of left pulmonary artery with part of tumor invading to the pulmonary trunk, prolapsing into its lumen and causing stenosis, with invasion of the upper lobe of the left lung was set. Simultaneous surgery under extracorporeal circulation: left-sided pneumonectomy with resection of the left pulmonary artery and pericardium was performed 31.03.2005.

Results. Postoperative period was satisfactory, patient was withdrawn from the hospital under observation of local oncologist. Considering absence of mediastinal lymph node involvement, clean bronchial and arterial resection margins, absence of distant metastases and low sensitivity to chemoradiotherapy of leiomyosarcoma combined treatment was not performed.

The patient was under observation for 7 years. She was complaining only for dyspnoea on slight exertion. Everyday exertion did not cause discomfort.

Conclusion. The introduction of cardiosurgery technique to thoracic oncology practice make resections and reconstructions of major intrathoracic vascular structures possible. Surgical treatment under extracorporeal circulation gave an opportunity to make radical operation to patient with pulmonary artery leiomyosarcoma and achieve full recovery.
