The method of improving surgical approach for gastric cancer

The development of gastric surgery stimulates advancement of instrumental techniques for surgical approach to organs particularly to stomach.

In Perm regional oncological dispensary new instrumental method of improving surgical approach for surgical treatment was applied in 50 patients. ЕА Gierev and VV Ferapontov supposed a new wound retractor (patent for invention No. 2363401 under 10.08.2009) which was a technical advancement of fered Sigal’s retractor, famous and wide-spread in surgical practice.

Comparative analysis of objective criteria for quality of surgical approach using different instrumental methods of improving surgical approach for gastric cancer was performed.

Material and methods. Comparative analysis of wound «window» measurements for different instrumental methods creating surgical approach in 50 patients prior to gastrectomy was performed. Patients were divided into study and control groups. The depth of wound, angle of surgical action, pitch angle of surgical action were assessed.

Results. In the study group with new retractor the depth of wound decreased by 1.5 cm and accounted for 63.02±2.07 cm. The angle of surgical action in the study group increased by 7 degree and pitch angle of surgical action increased by 1 degree comparing with the control group (р<0.05), thus, improving surgical approach and facilitating movements of surgeon.

Conclusion. The developed retractor improves surgical approach and its quality.
