Молекулярно-генетические исследования при выборе тактики хирургического и химиотерапевтического лечения рака щитовидной железы

Представлены результаты молекулярно-генетических исследований, проведенные разными исследовательскими группами и оценивающие перспективность анализа молекулярно-генетических маркеров у пациентов с различными формами рака щитовидной железы.

Мутации генов при раке щитовидной железы. Проведен анализ данных литературы по молекулярно-генетическим нарушениям при раке щитовидной железы, в которых показана прогностическая и диагностическая ценность таких молекулярно-генетических маркеров, как ген BRAF, KRAS, RET, а также другие гены, вовлеченные в патогенез рака щитовидной железы. В этих работах рассматривается возможность исследования мутаций в горячих точках генов BRAF и RET для выбора тактики оперативного вмешательства, а также исследование RAS-генов для определения ранних стадий трансформации ткани в злокачественное новообразование.

Приведены данные собственных исследований, которые в целом коррелируют с данными мировой литературы. Показано, что проводить исследования биологического материала можно как в образцах опухолевой ткани, так и в биологических жидкостях (плазма крови, слюна). Проведен анализ таких молекулярно-генетические маркеров, как BRAF, KRAS и TP53.

Заключение. Возможности молекулярно-генетического анализа позволяют использовать избирательный подход к лечению больных с разными типами рака щитовидной железы. При подходе, основанном на анализе генетических изменений в опухолевых клетках, становится возможным снизить риски осложнений при операции, уменьшить количество рецидивов и метастазирования, реализовать индивидуальный подход при химиотерапии и облучении. Новые терапевтические стратегии, основанные на использовании различных молекулярных механизмов, открывают возможности персонализированного таргетного лечения.
