Инвазивная сонография в диагностике опухолевой патологии поджелудочной железы: методика и результаты

Цель исследования: оценить диагностическую точность и безопасность биопсий под контролем трансабдоминального ультразвукового исследования (УЗИ) в диагностике опухолей поджелудочной железы (ПЖ).

Материалы и методы. Проведено ретроспективное исследование 52 биопсий под контролем УЗИ пациентам, поступившим на этап хирургического лечения в отделение абдоминальной хирургии МНИОИ им. П.А. Герцена в период с 2011 по 2013 гг. Биопсии проводили биопсийными иглами 18G и 20G. Полученный материал направляли на гистологическое или цитологическое исследование. Оценивали диагностическую точность и частоту осложнений. Всем пациентам было проведено хирургическое вмешательство в срок от 2 до 74 дней (31±7,4 дня) после проведения биопсии. Во время интраоперационной ревизии проводили оценку постманипуляционных осложнений и диссеминации по брюшине.

Результаты. Точность биопсии опухолей ПЖ составила 98%. Повторная биопсия потребовалась 2 пациентам (3%). У 50 пациентов морфологическое подтверждение диагноза было установлено после первой биопсии. Биопсийный материал был направлен на гистологическое исследование – у 40 (80%), на цитологическое исследование – у 10 пациентов (20%). Неинформативный материал получен только у 1 пациента. Частота осложнений составила 1,9%, частота выявления перитонеальной диссеминации – 7,6% (4 пациента).

Заключение. Биопсия опухолей ПЖ под контролем УЗИ является высокоэффективным и безопасным методом диагностики новообразований ПЖ.
