Experience of two-stage pancreaticoduodenal resection

The special techniques of pancreaticoduodenal resection in patients with malignant tumors of biliary-pancreaticoduodenal area with cardiovascular and respiratory diseases, and in patients with locally advanced tumors of adjacent organs.

Material and methods. 30 pancreaticoduodenal resections with reduced reconstructive stage of surgery performed from 1987 to 2011 in the department of thoraco-abdominal surgery of Rostov Cancer Research Institute were analyzed respectively.

Results. According to localization primary tumors were as follows: head of pancreas – 17, major duodenal papilla – 4, duodenum – 1, locally advanced gastric and colon cancer – 6 and 2 cases, respectively. All patients had adenocarcinomas of different differentiation. There were 18 males and 12 females, the average age was 57.4 (from 32 to 75).

The patients with indications for pancreaticoduodenal resections with reduced reconstructive stage of surgery were defined.

The acute postoperative complications in study group accounted for 73.3%, 2 patients died. The delayed reconstruction was performed in 24 patients with no complications.

Conclusion. The pancreaticoduodenal resections with reduced reconstructive stage of surgery may be operation of choice for emergency treatment, allows to reduce the duration of surgery, decreasing the risk of cardiovascular and respiration complications in patients with initial pathology of these organs, delayed reconstructive surgery was not associated with increased number of complications and may be performed in patients with severe co-morbidity.
