Opportunities of reconstructive surgery after ultraradical mastectomy

The specifics of surgical methods for defect closure after extraradical mastectomy are reviewed in the article.

Material and methods: the outcomes of treatment in 52 patients with locally advanced or recurrent breast cancer received treatment in Yaroslavl cancer hospitals from 1998 to 2007 were analyzed retrospectively.

Results. 3-year survival rates observed in 50 patients were as follows – 44.3% – overall and 34.8% – recurrence-free. 5-year survival rates observed in 29 patients were as follows – 36.1% overall and 21.5% – recurrence-free. For defect closure TRAM-flap was used in 35 patients, latissimus dorsi flap – in 5 patients, pectoralis flap – in 3 patients, combination of two or more flaps – in 9 patients. For reconstruction of chest case the polyester mesh was used in 11 patients, titanium metal plates in 9 patients, silicone module in 3 patients. The average area of defect accounted for 988 square meters. In acute post-operative period 3 patients died. In 17 patients partial flap necrosis was observed.

Conclusion. Implement of reconstructive and plastic surgery into oncoilogical practice allowed to perform extraradical mastectomis, use multiflap technique for defect closure. Accurate selection of patients and correct choose of method for defect closure provide good long-term outcomes in this group of patients.
