Small cell lung carcinoma on the background of Lambert Eaton myasthenic syndrome

A case of Lambert-Eaton myasthenic syndrome (LEMS) in men of 54 y.o. with 40 years’ experience of smoking with its manifestation ahead occult central cancer in lower lobe of left lung is reported. Upon reaching the remission of LEMS with corticosteroid therapy left-sided pneumonectomy, mediastinal lymphadenectomy followed by 6 courses of polychemotherapy in regimen etoposide and platinum were performed. Intraoperatively the central cancer in lower lobe of left lung stage рT1N2M0 IIIА with peribronchial growth was detected. Histological study proved small cell lung cancer. Timely combined control of autoimmune neuromuscular and neoplastic processes allowed to obtain complete remission of LEMS and absence of evident tumor progression during 3 years of follow-up. Detailed electrophysiological analysis of neuromuscular transmission disorders was performed allowing to differentiate myasthenia and LEMS; comparing sensitivity of different immunoassays and cancer markers in early diagnosis of lung tumor possibilities for immunohistochemical study to specify the histological type of lung cancer were found. Ethiological relationship between lung carcinoma and LEMS was proved by numerous evidences.
